Parent Information and Forms for Private Instructional Personnel (PIP) Requests

It is the desire of The School District of Lee County to work collaboratively with families and community providers to meet the unique needs of all students, specifically our students with disabilities in accordance with Florida Statute 1003.572.

All private instructional personnel desiring to observe, collaborate and/or provide services in The School District of Lee County for students with Individualized Education Plans must complete an annual application and submit for approval. All families and individuals desiring to complete an application for private instructional personnel to observe, provide services and collaborate with The School District of Lee County staff and students with Individualized Education Plans must complete a new application annually or if changes occur with private personnel or schools. Each application received will be good for the regular school year and instructional summer programs during the year in which it is completed if no changes occur during that year.

Updates for the 2024-2025 school year

The Private Instructional Personnel process was reviewed and there are changes in effect immediately for the 2024-2025 school year. These changes are in compliance with the state statute and are intended to support a more efficient process.

The updated process is online through the google platform. The Private Instructional Personnel process remains a two steps approval, the first approval of the process is to complete the provider request form, once providers are approved, they can then be added to the student request form. Please do not submit student requests until the providers have been approved. Refer to the information below regarding each of the steps of the approval process.

  4. Wait for Approval Email which will notify the parent/guardian, school based administration and the Private Instructional Provider Employer.

  5. Schedule a meeting with the Principal and Teacher, including the parent, and Private Instructional Provider with Supervisor if required for practice to discuss the students private plan of care and the agreed upon schedule for services on campus.

Provider Form – to be completed by the agency with the providers.

When completing the google form, please make sure to complete each question with accurate information. Please do not provide abbreviations for names of providers or companies. Additionally, please note that the following must be turned in with a provider form for it to be processed.

  1. Vendor Badges for EACH employee - Photocopy must include the provider name and the expiration date must be visible (Please DO NOT send a photocopy of the back of the vendor badges)
  2. Documentation of Professional Licensure - Supervision MUST be listed on the provider certification; if supervision is a requirement for active status, the supervisor listed on the request and the certification must match.
  3. Proof of Employment - a letter on company letterhead stating that the employee works for the company, this must be signed. We DO NOT accept E-Verify or I-9 - Please DO NOT send confidential employee information to us.
  4. Signed Communication Agreement and Confidentiality Statement - See attachments below, signed agreement must be provided by each provider. All providers should be familiar with the written agreements in this document before being on campus, questions can be discussed with the school based staff.

*If your employees do not have a current vendor badge, please see the attached form below regarding vendor fingerprinting and/or visit this website - Provider requests should not be submitted until providers have vendor badges.

PROVIDERS MUST BE APPROVED BEFORE SUBMITTING A STUDENT REQUEST

Student request Forms – This request is to be completed by the agency with the Parent/Guardian OR by the Parent/Guardian.

When completing this google form, please make sure to complete each question with accurate information. Additionally, please note that the following must be turned in with a student form for it to be processed

  1. The Private Plan of Care or Services (Ex. Behavior Plan)
  2. Completed and signed Release of Information - allowing for shared collaboration between the private provider, supervisor and the school personnel. This can be either the Lee Co. Form, found below OR the private providers form - It must give permission for collaboration between the school staff and the outside company staff.
  3. Completed and signed Communication Agreement and Confidentiality Statement from the parent/guardian (1 document, 3 pages) - Please do not resubmit the prover form.

Please only submit PDF documents in the google forms - other document formats may require resubmission.

For any document that requires password protection use “ LEE2024! ” - Please note that not all documents require password protection. Please DO NOT password protect all documents, only password protect those that contain confidential information.

Finally, it is helpful if you reduce the amount of attachments per submission – meaning please submit each employee “file” as one document instead of multiple.
