Vehicle Accident Involving a School Bus or District Vehicle
Please fill out the form and include your insurance policy number and insurance phone number if your vehicle was involved in an accident with a school bus or other district vehicle, including:
- Property damage
- Vehicle collisions
- Third-party driver incidents
Injury or Safety Incident Report
Please fill out the form if you or someone else was injured on school property or during a school-related activity, including:
- Slip and fall incidents
- Injuries during events
- Visitor injuries
- Student injuries
- All students have access to accident insurance coverage through the District’s KidGuard program. The clinic is responsible for submitting the accident insurance claim. Learn more about our Student Accident Insurance.
