COVID-19 Enrollment Questions

​The Student Enrollment Office is closed until further notice.  Enrollment personnel are available to assist virtually.  Please send email to studentenrollment@leeschools.net and include your child's/children's full name and date of birth. 

The following questions must be asked to every family coming to enroll or re-enroll into The School District of Lee County.

  1. Have you, or a member of your household traveled to, or been to one of the following Level 2 or Level 3 countries within the last 2 weeks?
    • China
    • Iran
    • Italy
    • Japan
    • South Korea
  2. Have you, or a member of your household been in direct contact with individual known to have COVID 19?
  3. Have you, or a member of your household been on a cruise ship within the last 2 weeks?
  4. If yes to questions 1, 2, or 3 Enrollment is to STOP.
  5. Family is directed to contact their primary care physician, or the Health Department for further instructions.
  6. Family of Enrollee, or Re-Enrollee is to self-quarantine for 14 days.
  7. Doctor is to provide clearance paperwork that family is to provide to Student Enrollment.
  8. Date must be at least 14 days after the initial visit.
