Elementary Proximity (Grandfathering) Form

1. View the new proximity map and determine if your current school is in your new proximity zone or out of your new proximity zone.

• If your current school is in your new proximity zone and you wish to remain there, you do not need to continue. You will automatically stay assigned to the school.

• If your current school is out of your new proximity zone, continue to step 2.

2. Log into the Focus Parent Portal or create a Focus Parent Portal account.

3. Choose Forms on the left and then choose Elementary Proximity.

4. On the form select the child, the school and the grade. Next, select your response and then hit Submit Request. Note: You will need to complete the form for each current elementary child in your family.

• If you select yes to keep your current school with no transportation provided by the District, you do not need to complete the Choice Application. You will automatically stay assigned to the school.

• If you select no and need to be assigned to a school in your new proximity zone, continue to the Elementary Choice Application 2023 Directions.
