Accident Insurance (Allstate)

The District offers an Accident Policy through Allstate.

This is a voluntary benefit offered to employees who are working at least 20 hours or more in a regular position.

Employees and qualified dependents can be covered. Eligible dependent children must be under the age of 26 when initially enrolled, and can be covered until the end of the calendar year in which they turn 26 years of age with NO Criteria (such as dependent marital status, student status, financial dependency on the Covered Employee, etc.) 

Dependent Eligibility

Spouse: Legally Married

Children: Employee's natural, newborn, adopted, foster, step child(ren) (or a child for whom the Covered Employee has been court-appointed as legal guardian or legal custodian) 

Accident coverage pays lump sum benefits to employees for on-and-off-the-job accidents for a variety of occurrences such as:

  • Dismemberment
  • Dislocation
  • Fracture
  • Ambulance Service
  • And More

Accident Insurance can help pay copays, deductibles and everyday living expenses.

There is a benefit of $25 per calendar year for a physician office visit.

Coverage is portable. If you leave the district this benefit can be continued as an individual plan at the same level of coverage when the insured terminates or retires.

Accident Insurance Plan 1 - Premiums per paycheck (24-pay):

Employee Only: $3.72
Employee/ Spouse: $6.43
Employee/ Child(ren): $8.25
Employee/ Family: $10.30

ATTENTION 20 PAY EMPLOYEES

PLEASE NOTE: The premiums listed above are based on 24 pay periods. Employee who receive 20 paychecks, 186-day work schedule, will pay the rates listed above with an additional 20% Employee Pre-Pay which will be applied toward summer coverage.

Employee Only: $4.46
Employee/ Spouse: $7.72
Employee/ Child(ren): $9.90
Employee/ Family: $12.36
