School Entry Requirements

A physical exam dated within 12 months prior to registration is required of all students K-12 except those transferring from any Florida public or private school. This must be done and signed by a physician, PA, ARNP or chiropractor.

Prior to entry, attendance or transfer to Florida schools (kindergarten through 12th grade), each child shall have on file a Florida Certification of Immunization (DH 680 form)

Exemptions

There are two types of exemptions for the immunization requirement: MEDICAL and RELIGIOUS. Exemptions will be granted for the following reasons:

  1. The parent of the child states that the administration of immunizing agents conflicts with his or her religious tenets or practices. This exemption must be granted by the Department of Health. The parent must call the Lee County Health Department to make an appointment for obtaining the religious exemption.
  2. A licensed physician certifies in writing, on a form provided by the Department of Health, that the child should be permanently exempt from the required immunization(s) for medical reasons.
  3. A licensed physician certifies in writing, on a form provided by the Department of Health, that the child is in the process of obtaining the required immunizations.
  4. The Department of Health determines that, according to recognized standards of medical practice, any required immunization is unnecessary or hazardous.

In the event of a communicable disease emergency, school health and immunization records of non-immunized students will be made available to the Lee County Health Department Director. Those students identified as not being immunized against the disease for which the emergency has been declared shall be temporarily excluded from school until such time as is specified by the Lee County Health Department Director.
