The newly formed
District Advisory Committee (DAC) has established an Executive Committee
to assist in the work of the DAC and to aid in the development of
effective local School Advisory Committees (SAC).
Accordingly to
facilitate the smooth flow of information from the SAC and interested
citizens to the DAC, the Executive Committee has established the
following procedure to aid all citizens in presenting items for review to
the DAC.
Send in
Your District advisory Council
Review/Request Form
We invite interested
citizens, School Advisory Committees and their members to complete our
online
“DAC
Review / Request Form”. When completed, it will be
automatically submitted to
Susan Johnson,
staff liaison for the DAC or you can mail a printed copy of the form to:

The
review / request
will be forwarded to the Executive Committee where it will be reviewed
with recommendations made to the District Advisory Council for hearing or action. The
Executive Committee may make referrals to other departments, personnel
or appropriate committees within the district for assistance.
Return your form as soon as possible
to allow the Executive
Committee and the District Advisory Committee an opportunity to review
your issue and assist in meeting your
objectives.
Be sure to
include your contact information and phone number so the District
Advisory Council Executive
Committee may contact you personally to discuss the item listed on the
review form. The Executive Committee will review your
DAC Review / Request Form and you will be advised in writing as to the
disposition of your request.
